How Our Integrated Model Works
The pathway
Initial consultation. We talk with the person's team, family and referrer about what is happening, what has been tried and what would help. Together we agree who needs to be involved and where to start. There is no obligation to go further.
Assessment. Where appropriate to the individual's needs, the right clinicians assess together rather than separately. This may include psychology, occupational therapy, speech and language therapy and PBS functional assessment, and avoids repeated referrals and duplicated questions.
Shared formulation. The team brings its findings into one picture of the person: their strengths, needs, communication, sensory experience and what may be keeping difficulties going. We share it with the people who support them, so everyone understands the person in the same way.
Intervention. From the formulation, we agree a practical plan. This may include direct therapeutic or OT work, a PBS plan, staff consultation, reflective practice and training, depending on what will help most.
Review. We review progress with you at agreed points, check that recommendations are working in daily life, and step support up or down as needs change.
What this means for you
One plan, not several reports. Recommendations are joined up and written in plain language for the people who deliver daily care.
Proportionate support. You only have the input that is needed, and it can change over time.
Practical advice. Every NEST clinician has worked in frontline roles, so recommendations are written to work in real settings.
A clear contact.
